Fortunately the 4th official generally has nothing to do than hold up a few boards. Clearly the Prem will probably having him cleaning the toilets before he leaves - which is probably just about his level.

I'm relieved he's only the fourth official. However, if we're a goal up on 90 minutes, expect more than the usual amount of injury time. In addition to stretching time, he can slow down substitutions and even get BFS or others on the touchline sent off. I expect nothing less than some unprecedented officiating from the 4th seat.
